Two things you'll need before we can start.
A physical office address
Insurance companies require a PHYSICAL address to serve as your office space. You can still offer hybrid services (telehealth and in-person), but you must have a physical location — it will be listed in the insurance company's online directory. Please don't fill out an application until you've determined your location.
Your business entity, decided
Before credentialing begins, your business entity must be determined — sole proprietor, LLC, PLLC, or S-Corp. This isn't something our team can decide for you: meet with a tax professional, since certain states require dietitians to be a specific entity to participate with insurance.
Why does the entity matter so much? We credential you under your business's EIN. If your EIN changes later, the insurance companies treat it as a brand-new business — and the entire credentialing process starts over from scratch.
